This application allows a provider to receive referrals from patients and the makes the referral connections. The current implementation focuses on smoking cessation.
As technology advances and becomes a part of our everyday activity, the potential for healthcare and patient engagement in preventative and post-discharge care becomes extremely feasible. Within the healthcare industry, there has been a paradigm shift towards the incorporation of technology to improve services and patient outcomes.
The proposed technology uses blockchain technology to connect individuals to services outside of the normal healthcare setting. Healthcare professionals will be able to engage patients outside of the office and engage long-term clinical care providers such as counseling centers, rehab facilities, and skilled nursing homes. These are facilities where a patient needs to spend time to get the continued care that is intended post-discharge. The proposed technology is currently focused to assist in connecting individuals with programs to help them stop smoking but this could be applied to help individuals with other cases. Also, the technology could work to connect individuals to healthcare services that meet their needs.
